Immigration Physicals

Chapel Hill Family Medicine is pleased to offer Medical Examinations for Immigration to Canada, Australia and New Zealand.
Dr. Burgert and Dr. Lambert are Designated Medical Practitioners who are certified by these countries to perform your Medical Exam.

Our Staff and Physicians have over 25 years of experience in providing this service. We look forward to serving you. Our aim is to make the process a pleasant and efficient experience for you.

Canadian Immigration

If you are applying for Canada Immigration you will need to bring the items listed below:


• Passport or copy of passport certified by a Canadian Visa Office
• List of current medications with dosage information

•Any vaccinations records you may have

 • Eye glasses or contact lenses, if you wear them
• Medical reports or test results that you have regarding to previous or existing medical conditions, and

 • If you are being sponsored, Canada Immigration should have sent an email with an attached form containing an IME number, or UCI number. You will need to bring this. If you are applying through the Express Entry program, you will not have a form.

Australian Immigration

If you are applying for Australian Immigration you will need to bring the items listed below:

• Your Referral Letter with your HAP ID
• List of current medications with dosage information
• Passport
• Your eyeglasses or contact lenses, if you wear them
• Any medical reports or test results regarding previous or existing medical conditions

•Any vaccinations records you may have

Immigration Form

New Zealand Immigration

If you are applying for New Zealand Immigration you need to bring the items listed below:

• Passport
• Eyeglasses or contact lenses, if you wear them
• A list of any medication you are taking (including dosage)
• Any medical reports or test results that you have regarding to previous or existing medical conditions
  • You may have received an email with a designated NZER number. Please provide us with this number when you are scheduling your examination.

Cost and Method of Payment:

Payment for your exam is only accepted in forms of cash, money order, or cashier's check. Credit cards / debit cards / personal checks are not accepted form of payments. We do NOT keep change if office, please bring exact change.


When scheduling your examination, please ask how much your appointment will cost.
